OK - at the bottom of this posting I have attached all the forms we need for PTLLS. City & Guilds have produced a single booklet with them all in (in Word format, so you should all be able to download these, and print / fill them in electronically as needed).
Just to help you navigate them:
Appendix 1 Form 1 - ignore - it's for your marker
App. 1 Form 2 - you fill in for each assignment to say it's your own work; hand it in; your marker adds feedback
App. 1 Form 3 - you use this for Practical Assignment 1 - it's for your scheme of work for 6 sessions
App. 1 Form 4 - you use this for Practical Assignment 2 - it's for your teaching session plan
App1. Form 5 - no-one in their right minds can work out what this form is actually for! It seems that you have to fill it in as a self-assessment of your own lesson plan that you put on Form 4 - which is a weird task and not actually mentioned in the assessment guidelines! But we'll all do it anyway.
App1. Form 6 - ignore - the person who comes to observe you teaching fills this one in
App1. Form 7 - you fill this form in with your own self-evaluation of your teaching (done for the same session you were observed in)
App1. Form 8 - we will use when we do micro-teaching in class to give each other feedback
App1. Form 9 - is the one you fill out right at the end of the course with your own personal action plan for your future development as a teacher (we do this together in a tutorial)
Appendix 2 - we fill out together when you have the tutorial at the start of your PTLLS course
Appendix 3 - we fill out as when you have tutorials
Appendix 4 - your tutor fills out as and when you complete each assignment
Appendix 5 - this is a really important one! This is the one you fill out and hand in with each of your assignments as your reflection.
Appendix 6 - you sign this at the end of the course to say everything was your own work, and your assessor(s) sign it to say you passed.
